Introduction: Periodontitis is a chronic inflammatory disease associated with subgingival biofilm dysbiosis and the destruction of tooth-supporting tissues. Ozone therapy has emerged as a complementary approach due to its antimicrobial properties and potential anti-inflammatory effects. Objective: To analyze the efficacy and clinical potential of ozone therapy as a non-invasive adjunctive treatment for periodontitis, considering its biological mechanisms and application modalities. Materials and Methods: A narrative review of publications from 2010 to 2025 was conducted using PubMed/MEDLINE, Scopus, SciELO, and Web of Science. DeCS/MeSH descriptors and Boolean operators were employed. Clinical trials, systematic reviews, meta-analyses, and experimental studies addressing the periodontal application of ozone were included, while publications with incomplete information were excluded. Results: The reviewed evidence suggests that ozone exhibits antimicrobial activity and potential anti-inflammatory effects. Its gaseous, aqueous, and oily applications have shown variable outcomes. Some studies reported reduced probing depth and improved clinical attachment levels when combined with scaling and root planing. However, methodological heterogeneity and limited long-term follow-up restrict the interpretation of its efficacy. Conclusions: Ozone therapy represents a promising complementary approach to non-surgical periodontal treatment, with the potential to improve specific clinical parameters. Nevertheless, further research using standardized protocols and longitudinal follow-up is required to confirm its long-term efficacy and safety.
